the antibiotic arc

october through december was the 'conservative management' era. antibiotics, warm sitz baths (sitting in a tub of warm water — undignified, weirdly soothing), keeping the area clean and hair-free, and a lot of hoping. and to be fair: it worked. the swelling went down, the pain faded, and for about three glorious weeks i forgot i had a tailbone at all.

then it came back. same spot, same ache, slightly worse attitude. an ultrasound at a delhi imaging centre confirmed what the second surgeon i consulted already suspected: the sinus tract was still there under the skin. antibiotics calm the infection, but they cannot close the tunnel. that's the honest mechanics of this condition.

antibiotics are a ceasefire. surgery is the peace treaty.

i want to be really clear about something, because i needed to hear it back then: a recurrence is not because you cleaned wrong or sat wrong or failed at healing. pilonidal sinus recurs because the tract physically exists. no amount of discipline dissolves a tunnel. if your doctor says the word 'recurrent', you haven't failed a test — you've just graduated to the next chapter.

by december i'd had two flare-ups, spent about ₹8,000 on the merry-go-round, and made my decision somewhere in a hot bath: i was done managing. i wanted it gone. which meant surgery, which meant finding the right person to do it — and that search changed everything.

✶ NOTE TO YOU, FUTURE SITTER
sitz baths, a donut cushion, and loose cotton clothing got me through the flare-ups. total cost of all three: less than one ultrasound. comfort is cheap, stock up.
